If you do not receive the form within 3 business days, please contact the CAMU at (866) 529-1890 and select option 1 or email camu@health.ny.gov.
HCS offers practitioners the ability to order Official New York State Prescription forms quickly and easily.
Important Note: Prescribing practitioners must be currently registered with the NYS Official Prescription Program.
