This submission form is to be utilized by certification candidates that completed an EMS provider course through a New York State-approved Course Sponsor and opted to complete the National Registry of EMTs (NREMT) in lieu of the New York State certification exam.

Please note: No submissions will be processed until after your End of Course date has passed. If you are not familiar with your course's End of Course date, please contact your instructor. This form must be filled out by the candidate as your legal signature verified by a government-issued photo ID is required.
