This form is for New York State students who opted to take the National Registry cognitive exam in place of the NYS exam. 

No submissions will be processed until after the official End of Course Date for the course you were enrolled in and until all course paperwork has been submitted and processed. 

All items requested by the this form must be submitted.

This form must be filled out by the student, and only the student. A signature and photo ID will be required.
