County EMS Plans
EMS Agency Data Collection
Pursuant to New York State Public Health Law § 3019 and General Municipal Law § 122-b, each county is required to develop and submit a Comprehensive County EMS System Plan to the Regional and State EMS councils and New York State Department of Health. The Department has developed a standardized County EMS Plan Framework to assist counties in creating comprehensive, collaborative, and data-informed plans that evaluate the current EMS system, identify service gaps, and establish strategies to strengthen the delivery of emergency medical services throughout each county.
To ensure that each county is best informed, each New York State EMS agency is requested to collect and provide data to the Department and the county or counties they provide service to. Provided with the County EMS Plan framework is a resource document (Attachment 1: EMS Agency Data Collection) for EMS agencies to utilize while they collect the required data metrics .
Once the EMS agency has collected all data metrics required, please select 'Next' to enter these data metrics into the survey. Your submission will be provided to both the Department and the county or counties in which the agency provides services.
Submissions to the portal are due no later than Sunday, August 9, 2026.
