 Action Description for DOH Webpage:License suspension is now terminated, and the physician is allowed to practice medicine with conditions for as long as he remains licensed in New York State and is subject to probation for five years with probation conditions. The physician’s New York State medical license is permanently limited to the practice of addiction medicine; he must successfully complete a fellowship in addiction medicine; practice only as part of the fellowship in addiction medicine and may only practice elsewhere following the successful completion of the fellowship in addiction medicine. The physician may not commence the active practice of medicine in New York State until all proposed monitors have been approved by the Director (Director) of the Office of Professional Medical Conduct.
 Misconduct Description for DOH Webpage:This order is a modification of the terms and conditions previously imposed under BPMC Order #17-150 that went into effect on June 2, 2017 and does not constitute a new disciplinary action. Previously the physician did not contest the charge of committing professional misconduct by having been convicted in the Supreme Court, State of New York, County of Ontario of Unlawful Imprisonment and Resisting Arrest.
  License Limitations or Conditions for DOH Webpage: The physician’s New York State medical license is permanently limited to the practice of addiction medicine. During the period of probation, the physician may only practice medicine in a group setting when his practice is being monitored by qualified health care professionals and supervised by a licensed physician, on-site at all locations. The physician is restricted from solo practice; may neither self-treat nor self prescribe any medications and shall only treat or prescribe medications to patients with whom he has a legitimate physician/patient relationship and for a legitimate medical purpose. The physician shall enroll in and successfully complete a minimum of fifty hours of Category 1 Continuing Medical Education each year in the physician’s specialty; continue in group and individual therapy with qualified health care professionals; submit to independent evaluations approved by the Director and shall continue enrollment in the Committee for Physician Health.
