 Action Description for DOH Webpage:License suspension for a minimum of twelve months and the physician must submit to a medical and psychiatric evaluation to determine the physician's fitness to practice medicine. Upon the determination that the physician is fit to practice medicine the suspension will be lifted and the physician will be subject to terms of probation for five years. The physician must provide a ninety days' notice before returning to New York State to practice medicine and shall recommence practice only upon the approval of the Director of the Office of Professional Medical Conduct.
 Misconduct Description for DOH Webpage:The Review Board affirmed the Hearing Committee's Determination finding the physician guilty of having been disciplined by the New Hampshire Board of Medicine for providing false information on her New Hampshire license renewal application.The Review Board affirmed the Hearing Committee's penalty but did modify the Hearing Committee's procedure for evaluating the physician's fitness to practice medicine.
  License Limitations or Conditions for DOH Webpage: The physician must provide ninety (90) days notice to the Director of the Office of Professional Medical Conduct of her desire to return to practice medicine in New York State.The physician shall recommence the practice of medicine only when approved by the Director of the Office of Professional Medical Conduct. During the term of probation the physician may only practice medicine in New York State when her practice is being supervised by a physician previously approved by the Director of the Office of Professional Medical Conduct.
