| | Misconduct Description for DOH Webpage: | Effective December 24, 2014 the Review Board issued a supplemental determination and order modifying the penalty issued on November 26, 2014 whereby the physician's license is restricted from prescribing controlled substances for one year beginning November 26, 2014. The Review Board affirmed the Hearing Committee's determination of August 4, 2014 finding the physician guilty of having been convicted in the Supreme Court for the State of New York, New York County of the Criminal Sale of Prescriptions for a Controlled Substance and modified the Hearing Committee’s penalty with suspension for two years, stayed and a license limitation restricting the physician from prescribing controlled substances for one year. Previously on February 25, 2014 the physician’s New York State medical license was summarily suspended by the New York State Commissioner of Health. |
