 Misconduct Description for DOH Webpage:The physician had not contested the charge of having been disciplined by the Wisconsin State Medical Examining Board for practicing medicine while impaired and for having been a habitual user of narcotics.Previously on February 8, 2013 the physician was subject to an interim non-disciplinary order of conditions whereby the physician agreed to not practice medicine until the final disposition of the New York State Office of Professional Medical Conduct investigation.
  License Limitations or Conditions for DOH Webpage: During the period of probation the physician may only practice medicine when he is being monitored by qualified health care professionals and his practice is being supervised by a licensed physician board certified in an appropriate specialty and who is on-site at the physician's practice location.
