The physician must provide ninety (90) days notice before practicing medicine in New York State or in any other jurisdiction where the practice of medicine is predicated on his New York State medical license after which his practice may be subject to conditions. The physician has paid the $1,500 fine and has inactivated his New York State medical license. The physician has also been granted a termination of the conditions imposed under BPMC# 12-269 effective March 14, 2016. |
