 Action Description for DOH Webpage:Censure and reprimand with probation for three years and a $2,500 fine. The physician is permanently restricted from prescribing any controlled substances. The physician completed the terms of his order effective February 23, 2014. The permanent restriction prohibiting the prescribing of controlled substances has been terminated and the physician is now subject to conditions. These conditions remain in effect for as long as the physician is licensed to practice medicine in New York State and include his compliance with Article 33 of the Public Health Law and he shall meet with the Office of Professional Medical Conduct (OPMC) for compliance; maintain a prescription log in a format acceptable to OPMC subject to review by the Director of OPMC and must make both medical and office records available for review by OPMC. All remaining terms of the original board order remain in effect.
 Misconduct Description for DOH Webpage:This order is a modification of the terms previously imposed on February 24, 2011 and does not constitute a new disciplinary action. Previously the physician had not contested the charge of having been convicted in the Criminal Court of the City of New York, Kings County of offering a false instrument for filing.
