| | Misconduct Description for DOH Webpage: | The Administrative Review Board affirmed the Hearing Committee's September 10, 2009 penalty and determination finding the physician guilty of negligence on more than one occasion; incompetence on more than one occasion and failure to maintain accurate patient records. Previously on April 24, 2009, the physician's New York State medical license was summarily suspended by the New York State Commissioner of Health. Later, on February 21, 2018 the New York State Education Department Board of Regents denied the physician's petition for the restoration of his New York State medical license. |
