| | Misconduct Description for DOH Webpage: | The Hearing Committee sustained the charge finding the physician guilty of having been disciplined by the Arizona Medical Board for gross negligence; repeated negligence or negligence resulting in harm to or death of a patient and failing to maintain adequate patient records. Previously on September 20, 2007 the physician had agreed to not engage in the practice of medicine in New York State until the final disposition of the current investigation by the Office of Professional Medical Conduct. |
