 Action Description for DOH Webpage:License limitation precluding the registration or issuance of any further New York State license. The physician assistant shall be precluded from practicing as a physician assistant in New York State, and from practicing in any setting where their practice is based solely on their New York State license, and from further reliance on their license to practice as a physician assistant to exempt them from the licensure, certification or other requirements set forth in statute or regulation for the practice of any other profession licensed, regulated or certified by the New York State Board of Regents; New York State Department of Education: New York State Department of Health or the New York State Department of State. This modification order also strikes the requirement to maintain active registration and payment of fees with the New York State Education Department. The physician assistant completed the previously imposed probation effective July 30, 2011.
 Misconduct Description for DOH Webpage:This order is a modification of the terms and conditions previously imposed under BPMC Order#08-133 that went into effect on July 31, 2008, and does not constitute a new disciplinary action. The physician assistant did not contest the charges of practicing as a physician assistant with gross negligence and incompetence.
