| | Misconduct Description for DOH Webpage: | The physician assistant did not contest the charges of having been convicted in Saugerties Town Court, Ulster County on April 9, 2008 of a violation for making official New York State prescriptions for scheduled substances; on April 14, 2008 in the Queensbury Town Court, Warren County for obtaining controlled substances by fraud and deceit; and on April 22, 2008 by a New York State Department of Health Stipulation and Order based on prescribing controlled substances in the name of another for her own use. Previously on June 13, 2007 the physician assistant was issued an Order not to Practice Medicine pending final disposition of an investigation. |
