 Misconduct Description for DOH Webpage:This order is a modification of the terms and conditions previously imposed under BPMC Order #03-273 that went into effect on October 29, 2003 and does not constitute a new disciplinary action. The physician did not contest the specifications, in full satisfaction of the charges of misconduct, by practicing the profession of medicine with negligence on more than one occasion and failing to maintain accurate patient medical records.
