 Action Description for DOH Webpage:The physician's petition for restoration is denied but the execution of the license revocation is stayed in full.  Misconduct Description for DOH Webpage:Previously the New York State Board for Professional Medical Conduct Hearing Committee sustained the charges finding the physician guilty of negligence on more than one occasion; gross negligence; practicing fraudulently; failing to maintain adequate records; failing to complete forms required for the reimbursement of a patient or third party and harassing, abusing or intimidating a patient. The Review Board on October 8, 2002 remanded this matter to the Hearing Committee to clarify their order and determination.On December 17, 2002, the Hearing Committee issued an amended Hearing Committee Report sustaining their previous determination and penalty. The New York State Supreme Court, Appellate Division confirmed the revocation of the physician's New York State medical license on April 8, 2004. The New York State Department of Education Board of Regents stayed the June 19, 2002 revocation of the physician's New York State medical license and placed the physician on probation for two years with conditions and restrictions.Upon the physician's successful completion of the probationary period, the physician's medical license will be restored.
  License Limitations or Conditions for DOH Webpage: During the period of probation the physician may only practice medicine as a salaried physician in a hospital or a facility regulated under New York State Health Law Article 28 or with the Veterans Administration or in another group setting pre-approved by the Director of OPMC.The physician shall not engage in a solo or private practice.The physician may not perform unsupervised surgery.
