| | Action Description for DOH Webpage: | Censure and reprimand and the physician’s New York State medical license shall permit them to prescribe, dispense, order and/or administer non-controlled substances and shall be limited to permit them to prescribe, dispense, order and/or administer Propofol, benzodiazepines, Ketamine, inhaled anesthetics, muscle relaxants, antiemetics, anti-inflammatory and non-opioid topicals, only in a setting license under Article 28 of the Public Health Law. Any patients being prescribed controlled substances other than those listed and/or other than in a setting licensed under Article 28 of the Public Health Law prior to the execution of this agreement, shall upon execution of this agreement, be referred to another health care provider who can exercise their independent judgement and discretion regarding continuing and/or modifying their necessity and/or use of controlled substances. The physician shall enroll in and successfully complete Continuing Medical Education approved by the Director of the Office of Professional Medical Conduct. The physician must also fully comply with the 2022 and 2024 Florida Board of Medicine Board Orders and any extension or modification thereof. |
