 Misconduct Description for DOH Webpage:The physician asserted they could not successfully defend against at least one act of misconduct alleged, in full satisfaction of the charges of misconduct by practicing the profession of medicine with negligence on more than one occasion and failing to maintain accurate patient medical records regarding two patients.
  License Limitations or Conditions for DOH Webpage: The physician is permanently precluded from treating and/or prescribing, dispensing, ordering and/or administering any controlled substances to their family, friends, or any person with whom the physician does not have a bone-fide physician patient relationship. During the period of probation the physician shall enroll in and successfully complete Continuing Medical Education approved by the Director (Director) of the Office of Professional Medical Conduct; shall only practice medicine when their practice is being monitored by a licensed physician, board-certified in an appropriate specialty; must comply with all requirements of Article 33 of the Public Health Law and consultation of iStop when writing prescriptions for Schedule II, III, and/or IV controlled substances as well as maintenance of a controlled substance log.
