 Misconduct Description for DOH Webpage:The physician did not contest the allegations, in full satisfaction of the charges of misconduct, by having been disciplined by the Louisiana Board of Medical Examiners for concerns regarding the legibility and adequacy of documentation; frequency of patient visits; lack of adequate justification for lab testing; lack of adequate justification for excessive use of injections in violation of the Louisiana Medical Practice Act and for unprofessional conduct including, but not limited to, failing to create or maintain medical records, medical incompetency, and violation of any rules and regulations of the board or any provision of the Louisiana Medical Practice Act.

During the period of probation, the physician shall only practice medicine when their practice is being monitored by a licensed physician, board-certified in an appropriate specialty and must enroll in and successfully complete Continuing Medical Education approved by the Director.
