 Action Description for DOH Webpage:Censure and reprimand and should the physician decide to resume the practice of medicine in New York State, before beginning such practice, they must provide ninety days advance written notice to the Director (Director) of the Office of Professional Medical Conduct, receive the approval of the Director and may be subject to further conditions imposed by the Director. Upon the physician’s return to practice in New York State, the physician shall enroll in and successfully complete Continuing Medical Education (CME) approved by the Director. The physician shall enroll in the first available CME within sixty days of return to practice and successfully complete the CME to the satisfaction of the Director including any extended courses that may be required.
 Misconduct Description for DOH Webpage:The physician did not contest the allegations, in full satisfaction of the charges of misconduct, by having been disciplined by the California Medical Board for failing to maintain the ability to provide compliant prescriptions for narcotic pain management and failed to recognize and/or utilize alternative strategies for providing a patient with a compliant prescription; suggested a narcotic addition without a factual basis and failed to review CURES in the administration of narcotic prescriptions.
