 Action Description for DOH Webpage:License suspension for three years, stayed in full. The physician shall be placed on probation for three years with terms that include a practice monitor and Continuing Medical Education. The physician’s New York State medical license shall be permanently limited to permit the physician to prescribe, dispense, order and/or administer non-controlled substances and the prescribing, dispensing, ordering and/or administering of Propofol, benzodiazepines, Ketamine, inhaled anesthetics, muscle relaxants, antiemetics, anti-inflammatory and non-opioid topicals, only in a surgical and/or procedural setting. Any patients being prescribed controlled substances other than those listed, and/or other than in a surgical or procedural setting prior to the execution of this Order, shall be referred to another health care provider who can exercise their independent judgement regarding continuing and/or modifying the necessity and/or us of these controlled substances.
 Misconduct Description for DOH Webpage:The physician asserted they could not successfully defend against at least one act of misconduct alleged, in full satisfaction of the charges of misconduct, by practicing the profession of medicine with negligence on more than one occasion and failing to maintain accurate patient medical records regarding five patients.
  License Limitations or Conditions for DOH Webpage: The physician’s New York State medical license shall be permanently limited to permit the physician to prescribe, dispense, order and/or administer non-controlled substances and the prescribing, dispensing, ordering and/or administering of Propofol, benzodiazepines, Ketamine, inhaled anesthetics, muscle relaxants, antiemetics, anti-inflammatory and non-opioid topicals, only in a surgical and/or procedural setting. During the period of probation, the physician may only practice medicine when their practice is being monitored by a licensed physician, board-certified in an appropriate specialty and must enroll in and successfully complete Continuing Medical Education approved by the Director of the Office of Professional Medical Conduct.
