Any patients being prescribed controlled substances prior to this order shall be appropriately referred to another health care provider who can exercise their independent judgement and discretion regarding continuing and/or modifying the necessity and/or use of controlled substances.
 Misconduct Description for DOH Webpage:The physician asserted they could not successfully defend against at least one act of misconduct alleged, in full satisfaction of the charges of misconduct, by practicing the profession of medicine with negligence on more than one occasion and failing to maintain accurate patient medical records regarding six patients.
