The physician shall also be placed on probation for three years with conditions including a practice monitor and Continuing Medical Education.
 Misconduct Description for DOH Webpage:The physician asserted they could not successfully defend against at least one act of misconduct alleged, in full satisfaction of the charges of misconduct, by practicing the profession of medicine with negligence on more than one occasion and failing to maintain accurate patient medical records.
