 Action Description for DOH Webpage:License suspension for thirty-six months, stayed in full. The physician shall be placed on probation for thirty-six months with conditions including a practice monitor and Continuing Medical Education. The physician’s New York State medical license is permanently limited to preclude the prescribing, dispensing, ordering and/or administering of controlled substances. Any patients being prescribed controlled substances prior to the execution of this Order shall be appropriately referred to another health care provider, who can exercise their independent judgement and discretion regarding continuing and/or modifying the necessity and/or use of controlled substances.
 Misconduct Description for DOH Webpage:The physician asserted they could not successfully defend against at least one act of misconduct alleged, in full satisfaction of the charges of misconduct, by practicing the profession of medicine with negligence on more than one occasion and failing to maintain accurate patient medical records for six patients.
