| Misconduct Description for DOH Webpage: | The Hearing Committee sustained the charges finding the physician committed professional misconduct by having been disciplined by the Commonwealth of Kentucky Board of Medical Licensure, for failing to conform to acceptable and prevailing medical practices including cut and paste notes with no meaningful subjective, objective, assessment and plan (SOAP) information; prescribing suboxone to patient without sufficient monitoring and counseling; failing to address inappropriate drug screen results; prescribing to patients positive for methadone; making no effort to taper and providing early refills. Previously on March 22, 2023, the physician’s New York State medical license was suspended by the New York State Commissioner of Health. |
