The physician’s New York State medical license is permanently limited to examining and/or treating female patients only in the presence of a chaperone subject to the approval of the Director (Director) of the Office of Professional Medical Conduct and maintenance of a chaperone log. The physician must also obtain an assessment from the Committee for Physician Health (CPH) and completed to the satisfaction of the Director. The physician may be subject to further conditions imposed by the Director based on this assessment. If the CPH assessment results in enrollment with CPH, the period of probation shall be modified to sixty months from the effective date of this board order with conditions including a practice monitor and Continuing Medical Education.
