 Action Description for DOH Webpage:License suspension for thirty-six months, with the first six months to be served as actual suspension and the last thirty months stayed. The physician shall then be placed on probation for thirty-six months with conditions including a practice monitor and Continuing Medical Education.  Misconduct Description for DOH Webpage:The physician asserted they could not successfully defend against at least one of the acts of misconduct alleged, in full satisfaction of the charges of misconduct, by practicing the profession of medicine with negligence on more than one occasion and failing to maintain accurate patient medical records regarding eight patients.
