 Action Description for DOH Webpage:The physician is now subject to a license limitation precluding the registration or issuance of any further New York State medical license. The physician is precluded from practicing medicine in New York State and from practicing in any setting where his practice is based solely on his New York State medical license. The physician is also precluded from further reliance upon his New York State medical license to exempt him from the licensure, certification or other requirements set forth in statute or regulation for the practice of any other profession licensed, regulated or certified by the New York State Board of Regents, New York State Department of Education, New York State Department of Health or the New York State Department of State. This modification order also strikes the condition that the physician maintain active registration and pay all related fees with the New York State Education Department as well as maintaining a Physician Profile.
 Misconduct Description for DOH Webpage:This action is not disciplinary in nature but modifies the previously imposed penalty issued under BPMC Order #20-276 that went into effect November 10, 2020. The physician asserted he could not successfully defend against at least one act of professional misconduct by practicing the profession of medicine with negligence on more than one occasion and failing to maintain accurate medical records.
