 Action Description for DOH Webpage:The physician has completed the Clinical Competency Assessment imposed under BPMC Order#20-202 that went into effect on August 13, 2020 and as of the effective date of this Director’s First Modification order, the preclusion from prescribing, dispensing, ordering and/or administering of controlled substances is now terminated. The roles and duties of the Preceptor and Practice Monitor in the probation terms shall remain separate and distinct, however, the same physician may act as both Preceptor and Practice Monitor subject to the written approval of the Director of the Office of Professional Medical Conduct. The physician remains subject to all other remaining probation terms and conditions imposed in the Original Board Order as well as the permanent license limitation requiring the presence of a chaperone when examining and treating female patients and may be subject to additional terms and conditions the Director may impose based on all relevant facts and circumstances known to the Director. Previously on August 17, 2012 the physician was subject to a permanent license limitation requiring the presence of a chaperone when examining and treating female patients.
 Misconduct Description for DOH Webpage:The physician asserted he could not successfully defend against at least one of the charges of professional misconduct alleged by practicing the profession of medicine with negligence on more than one occasion and failing to maintain accurate patient medical records.
  License Limitations or Conditions for DOH Webpage: Permanent license limitation requiring the presence of a chaperone when examining and treating female patients. During the period of probation the physician may only practice medicine when his practice is being monitored by a licensed physician board certified in an appropriate specialty. The physician remains subject to all other remaining probation terms and conditions imposed in the Original Board Order and may be subject to additional terms and conditions the Director may impose based on all relevant facts and circumstances known to the Director.
