| | Action Description for DOH Webpage: | The physician has completed the Clinical Competency Assessment imposed under BPMC Order#20-202 that went into effect on August 13, 2020 and as of the effective date of this Director’s First Modification order, the preclusion from prescribing, dispensing, ordering and/or administering of controlled substances is now terminated.
