 Action Description for DOH Webpage:Upon the effective date of this Modification Order, the permanent license limitation on their New York State medical license shall terminate. Should the physician decide to resume the practice of medicine in New York State, they must provide ninety days advance written notice to the Director (Director) of the Office of Professional Medical Conduct, receive approval from the Director and may be subject to further conditions imposed by the Director. The physician shall be placed on probation for three years with terms that include Continuing Medical Education and a practice monitor. All remaining terms and conditions will continue as written in the Original Order.
 Misconduct Description for DOH Webpage:This order is a modification of the terms and conditions previously imposed under BPMC Order #20-239 that went into effect on September 24, 2020, and does not constitute a new disciplinary action. The physician did not contest the allegations, in full satisfaction of the charges of misconduct, by having been disciplined by the State of New Jersey Department of Law & Public Safety, Division of Consumer Affairs, Board of Medical Examiners, for their care and treatment of six people for whom they prescribed Controlled Dangerous Substances (CDS).

During the period of probation, the physician shall enroll in and successfully complete Continuing Medical Education approved by the Director and may only practice medicine when their practice is being monitored by a licensed physician, board-certified in an appropriate specialty, and approved by the Director.
