 Action Description for DOH Webpage:License suspension for thirty-six months, stayed in full. The physician is placed on probation for thirty-six months with conditions and their New York State medical license shall be permanently limited to preclude the ordering, prescribing, administering, dispensing, or otherwise utilizing controlled substances or supervising other providers in doing so. The physician’s New York State medical license is also permanently limited to preclude the ordering, prescribing, administering, dispensing, or otherwise utilizing non-controlled substances or any modalities in the control or treatment of pain, or in supervising other providers in doing so, with specific exceptions. When medically indicated, the physician shall refer patients to another health care provider to evaluate or treat patients with complaints of pain which cannot be adequately managed consistent with the terms of this order. During the period of probation, the physician may only practice medicine when their practice is being monitored by a licensed physician, board certified in an appropriate specialty and must enroll in and successfully complete a Continuing Medical Education program approved by the Director of the Office of Professional Medical Conduct.
 Misconduct Description for DOH Webpage:The physician did not contest the allegations in full satisfaction of the charges of committing professional misconduct by practicing the profession of medicine with negligence on more than one occasion regarding seven patients.
