 Action Description for DOH Webpage:License suspension for three years, stayed in full. The physicians New York State medical license is permanently limited to preclude the prescribing, dispensing, ordering and/or administering of any controlled substances is now amended to allow the physician to provide this service in a setting licensed under New York State Public Health Law, Article 28 or other setting pre-approved by the Director of the Office of Professional Medical Conduct and only after consulting with another physician who shall co-sign the note in the medical record. The physician remains on probation for three years with conditions. The physician completed probation on December 24, 2023, however, the amended limitation on controlled substances remains in effect.
 Misconduct Description for DOH Webpage:This order is an amendment of the terms and conditions previously imposed under BPMC # 20-274 that went into effect November 5, 2020 and does not constitute a new disciplinary action. The physician asserted he could not successfully defend against at least one of the charges of professional misconduct alleged by practicing the profession of medicine with negligence on more than one occasion and failing to maintain accurate patient medical records.
