| License Limitations or Conditions for DOH Webpage: | During the period of probation the physician shall practice medicine only when monitored by a licensed physician, board certified in an appropriate specialty. The physician is permitted to prescribe Citalopram, Escitalopram, Fluvoxamine, Paroxetine, Fluoxetine, Sertraline, Desyrel, Buspirone and Venlafaxine. All other controlled substances and psychotropic medications can be prescribed, when medical necessary and after consultation from another physician within the practice. After consultation, he may prescribe no more than a five day supply of these medications, to carry the patient until they can be seen by a specialist and must refer these patients to a specialist as warranted by their condition. The physician must maintain a log of all ordering, prescribing, administering and/or dispensing of all controlled substances and psychotropic medications. The physician must maintain a log of all referrals made to any specialist, referral service or any consult sought, regarding patients for whom he prescribed controlled substances and/or psychotropic medications. He must successfully complete a continuing medical education program approved by the Director of the Office of Professional Medical Conduct. |
