 Action Description for DOH Webpage:Censure and reprimand and the physician’s New York State medical license is limited to preclude the provision of medical care to patients located in New York State via telemedicine and may only provide care to patients in person while physically present in New York State at a location and setting approved by the Director (Director) of the Office of Professional Medical Conduct. The physician is also placed on probation for thirty-six months with conditions; must provide ninety days advanced written notice and receive approval from the Director before resuming the practice of medicine in New York State and may be subject to additional conditions imposed by the Director. Previously on August 21, 2019, the physician’s New York State medical license was suspended by the New York State Commissioner of Health.
 Misconduct Description for DOH Webpage:The physician did not contest the charge of committing professional misconduct by having been disciplined by the Commonwealth of Kentucky’s Board of Medical Licensure by practicing medicine with gross incompetence; gross ignorance; gross negligence or malpractice and failing to comply with appropriate standards of medical practice or principles of medical ethics regarding at least three patients.
  License Limitations or Conditions for DOH Webpage: The physician’s New York State medical license is limited to preclude the provision of medical care to patients located in New York State via telemedicine and may only provide care to patients in person while physically present in New York State at a location and setting approved by the Director (Director) of the Office of Professional Medical Conduct. During the period of probation the physician may only practice medicine when his practice is being monitored by a licensed physician board certified in an appropriate specialty; must comply with the requirements of Article 33 of the Public Health Law when writing prescriptions for Schedule II, III and/or IV controlled substances and must maintain a prescript log of all ordering, prescribing, administering and/or dispensing of all controlled substances. The physician must also successfully complete a Continuing Medical Education program approved by the Director.
