| Action Description for DOH Webpage: | The physician's New York State medical license is limited precluding him from ordering, prescribing, distributing or administering controlled substances in the setting of a private practice or other non-institutional setting, whether owned or managed by the physician or others. The physician may also not cause other individuals to order, prescribe or administer controlled substances in a private practice setting or non-institutional setting. This limitation with the express prior written approval of the Director of the Office of Professional Medical Conduct does not preclude the physician from ordering, prescribing, distributing or administering controlled substances in an appropriate institutional setting.The physician is also placed on probation for thirty-six months with conditions. The physician completed probation on January 23, 2021, however, the license limitation precluding the physician from ordering, prescribing, distributing or administering controlled substances in the setting of a private practice or other non-institutional setting, whether owned or managed by the physician or others remains in effect. The physician may also not cause other individuals to order, prescribe or administer controlled substances in a private practice setting or non-institutional setting. This limitation shall not preclude the physician from ordering, prescribing, administering, or dispensing controlled substances in an appropriate institutional setting with the express prior written approval from the Director of the Office of Professional Medical Conduct. |
