 Action Description for DOH Webpage:License limitation precluding the registration or issuance of any further New York State medical license. The physician is precluded from practicing medicine in New York State and from practicing in any setting where their practice is based solely on their New York State medical license. The physician is also precluded from further reliance upon their New York State medical license to exempt them from the licensure, certification or other requirements set forth in statute or regulation for the practice of any other profession licensed, regulated or certified by the New York State Board of Regents, New York State Department of Education, New York State Department of Health or the New York State Department of State. This modification order also strikes the condition that the physician maintain active registration with the New York State Education Department and payment of fees as well as maintenance of their New York State Physician Profile. All remaining terms and conditions will continue as written in the Original Order. The physician complied with the Pennsylvania order on November 23, 2020.
 Misconduct Description for DOH Webpage:This order is a modification of the terms and conditions previously imposed under BPMC Order #17-336 that went into effect on December 12, 2017, and does not constitute a new disciplinary action. The physician did not contest the charge of committing professional misconduct by having been disciplined by the Pennsylvania State Board of Medicine for having a history of chemical abuse or dependence and opiate addiction.
