| Misconduct Description for DOH Webpage: | The Review Board affirmed the Hearing Committee's July 4, 2017 determination finding the physician guilty of having committed professional misconduct by having been convicted in the New York State Supreme Court, Queens County, of Grand Larceny in the second degree; Forgery in the second degree; Falsifying Business Records in the first degree and Offering a False Instrument for Filing in the first degree. The Review Board overturned the Hearing Committee's penalty and revoked the physician's license to practice medicine. Previously on July 4, 2017 the physician's license was suspended for one year, retroactively beginning on January 19, 2017 and subject to a permanent limitation, limiting her practice of medicine to Obstetrics and Gynecology. Upon termination of the license suspension, the physician will be placed on probation for three years with conditions and prior to the resumption of practice the physician must complete fifty hours of continuing medical education in Obstetrics and Gynecology. On December 21, 2016, the physician's New York State medical license was summarily suspended by the New York State Commissioner of Health. This Hearing Committee's Determination was appealed to the Administrative Review Board. |
