The physician is precluded from practicing medicine in New York State and from practicing in any setting where his practice is based solely on his New York State medical license. The physician is also precluded from further reliance upon his New York State medical license to exempt him from the licensure, certification or other requirements set forth in statute or regulation for the practice of any other profession licensed, regulated or certified by the New York State Board of Regents, New York State Department of Education, New York State Department of Health or the New York State Department of State. This modification strikes the condition that the physician maintain active registration and pay all fees with the New York State Education Department as well as maintain a Physician Profile.
 Misconduct Description for DOH Webpage:This order is a modification of the terms and conditions previously imposed under BPMC Order #16-353 that went into effect November 1, 2016 and does not constitute a new disciplinary action. The physician asserted that he could not successfully defend against at least one of the professional misconduct charges of negligence on more than one occasion and failure to maintain accurate records by failing to appropriately evaluate patients; inappropriately prescribing controlled substances by not documenting an adequate medical basis or appropriate monitoring plan and failing to refer patients for consultation with other treating professionals.
