 Action Description for DOH Webpage:Indefinite license suspension for a period of no less than thirty-six months.During the period of suspension the physician is precluded from practicing as a physician and from reliance upon his New York State medical license to exempt him from the licensure, certification or other requirements set forth in statute or regulation for the practice of any other profession licensed, regulated or certified by the New York State Board of Regents, New York State Department of Education, New York State Department of Health or the New York State Department of State. Upon a showing to the satisfaction of a Committee of Professional Medical Conduct that the physician is both fit and clinically competent to practice medicine, the suspension may be terminated and the physician will be placed on probation for no less than five years with conditions and may be subject to additional conditions determined by the Committee.
 Misconduct Description for DOH Webpage:The physician did not contest the charges of fraudulently obtaining his New York State medical license; practicing the profession of medicine fraudulently; willfully making or filing a false report or failing to file a report required by law or by the New York State Education Department and engaging in conduct in the practice of medicine that evidences moral unfitness.
  License Limitations or Conditions for DOH Webpage: The physician’s medical license is suspended indefinitely, for a minimum of thirty-six months, during which he cannot practice medicine in New York State. Should the State Board terminate the suspension, the physician is placed on probation for a minimum of five years. During the period of probation the physician may only practice medicine when his medical practice is being supervised by a licensed physician and when he is being monitored by a qualified health care professional.
