 Action Description for DOH Webpage:The physician has completed his Clinical Competency Assessment (CCA) and remains subject to probation with conditions for thirty-six months as indicated in the original board order. This modification order imposes additional terms and conditions of probation.
 Misconduct Description for DOH Webpage:This order is a modification of the terms and conditions previously imposed under BPMC# 20-067 that went into effect on March 24, 2020 and does not constitute a new disciplinary action. The physician did not contest the charge of committing professional misconduct by having practiced the profession of medicine with negligence on more than one occasion.
  License Limitations or Conditions for DOH Webpage: During the period of probation the physician may only practice medicine with a supervising physician ‘at the elbow’ for a minimum of six months. At the end of the six months, the supervising physician will prepare a comprehensive report outlining the physician’s progress, need for continued supervision and ability to practice independently. The physician must obtain the written approval of the Director prior to any return to independent unsupervised medical practice. The physician must propose an Educational Preceptor for the Director’s written approval, who will design an educational curriculum based on any deficiencies identified in the CCA and/or observed in his practice. The physician shall complete a minimum of fifty credits of Category 1 CME per year of probation approved in writing before registration. In the first year, the CME requirement is to be completed through the Med-Challenger CME program. Additionally, the physician shall complete a course in Medical Documentation approved in writing. The physician may only practice medicine when his practice is being monitored by a licensed physician board certified in an appropriate specialty and is subject to all remaining terms and conditions of the original board order. The Director reserves the authority to impose additional conditions based on all relevant facts and circumstances.
