 Action Description for DOH Webpage:This order is a modification of the terms and conditions previously imposed and subjects the physician to a license limitation precluding the registration or issuance of any further New York State medical license. The physician is precluded from practicing medicine in New York State and from practicing in any setting where his practice is based solely on his New York State medical license. The physician is also precluded from further reliance upon his New York State medical license to exempt him from the licensure, certification or other requirements set forth in statute or regulation for the practice of any other profession licensed, regulated or certified by the New York State Board of Regents, New York State Department of Education, New York State Department of Health or the New York State Department of State. This order strikes the condition that the physician pay all New York State registration fees and maintain a physician profile. The physician paid the previously imposed fine, however, all other remaining terms and conditions of the order remain in effect.
 Misconduct Description for DOH Webpage:This order is a modification of the terms and conditions previously imposed under BPMC Order #16-155 that went into effect May 16, 2016 and does not constitute a new disciplinary action. The physician agreed not to contest the charges of having entered into a settlement agreement with the New Hampshire State Board of Medicine for failing to adequately supervise a physician assistant during the course of his practice.
