 Action Description for DOH Webpage:The physician successfully completed the Clinical Competency Assessment as well as the terms of probation. The physician must now comply with all requirements of the New York State Public Health Law Article 33 when writing prescriptions for Schedule II, III and/or IV controlled substances and for a period of two years, must maintain a prescription log of all ordering, prescribing, administering and/or dispensing of all controlled substances in a format approved by the Office of Professional Medical Conduct. All remaining terms and conditions of the Original Board Order remain in effect.
 Misconduct Description for DOH Webpage:This order is a modification of the terms and conditions previously imposed under BPMC Order #19-001 that went into effect on January 9, 2019 and does not constitute a new disciplinary action. The physician asserted they could not successfully defend against at least one of the alleged charges of professional misconduct of practicing the profession of medicine with negligence on more than one occasion and failing to maintain accurate patient medical records.
