| | Misconduct Description for DOH Webpage: | The Administrative Review Board affirmed the Hearing Committee’s January 25, 2023 determination finding the physician committed professional misconduct by violating previously imposed terms and conditions of probation by knowingly making false representations with intent to mislead when completing a credentialing initial appointment application to the Nassau University Medical Center: providing false information regarding their employment status on multiple occasions, failing to provide a true work history, failing to provide professional addresses and failing to provide additions or changes to their employment information as required; failing to respond to multiple requests for written verification relating to their employment and failing to provide proof of proper medical malpractice insurance in the amounts required by the Office of Professional Medical Conduct. Previously on August 16, 2018, the Administrative Review Board affirmed the Hearing Committee’s April 5, 2018, determination finding the physician committed professional medical conduct by having been disciplined by the New Jersey State Board of Medical Examiners for failing to properly place an epidural catheter in one patient; failing to remove the catheter; failing to properly record their treatment of this patient and failing to advise anyone of the misplaced catheter for over one month. The Administrative Review Board modified the Hearing Committee's penalty by increasing the term of probation from one year to three years after completing a license suspension of fifteen months. |
