 Action Description for DOH Webpage:Censure and reprimand and the physician’s New York State medical license is permanently limited to practicing medicine in a group setting in New York State which shall have at least two other licensed physicians. The physician must complete a combined fifty hours of medical education on the topics of prescribing practices and recordkeeping prior to commencing practice in New York State. The courses are in addition to any existing licensing requirements and must be pre-approved by the Director (Director) of the Office of Professional Medical Conduct. The physician must submit proof of completion to the Director within thirty days of course completion and if and when the physician begins practicing medicine in New York State, they will be subject to probation for five years with conditions. Previously on March 27, 2019, the physician was subject to an Interim Non-Disciplinary Order of Conditions.
 Misconduct Description for DOH Webpage:This consent order modifies part of the penalty previously imposed on March 29, 2022 by the Hearing Committee and does not constitute a new disciplinary action. The Hearing Committee sustained the charges finding the physician guilty of committing professional misconduct by having been convicted in the U.S. District Court of New York, Western District of New York, of Identity Theft (two counts) and Possession of Hydromorphone HCI (one count) as well as practicing the profession of medicine with negligence on more than one occasion and failing to maintain accurate medical records.

During the period of probation, the physician may only practice medicine when their practice is being monitored by a licensed physician, board certified in an appropriate specialty and monitored by a qualified health care professional approved by the Director. The physician must also maintain a log of all ordering, prescribing, administering and/or dispensing of all controlled substances in accordance with this consent order.
