 Action Description for DOH Webpage:Indefinite license suspension for a period of no less than twelve months. During the period of suspension the physician assistant is precluded from practicing as a physician assistant and from reliance upon her New York State medical license to exempt her from the licensure, certification or other requirements set forth in statute or regulation for the practice of any other profession licensed, regulated or certified by the New York State Board of Regents, New York State Department of Education, New York State Department of Health or the New York State Department of State. Upon a showing to the satisfaction of a Committee of Professional Medical Conduct that the physician assistant is both fit and clinically competent to practice medicine, the suspension may be terminated and the physician assistant will be placed on probation for no less than five years with conditions and may be subject to additional conditions as determined by the Committee.
 Misconduct Description for DOH Webpage:The physician assistant did not contest the charge of having committed professional misconduct by having been convicted in the Suffolk District Court, County of Suffolk, State of New York of Driving While Ability Impaired by the Combined Influence of Drugs or of Alcohol and Any Drug or Drugs and for having been convicted in the County Court of the State of New York, Nassau County of Driving While Ability Impaired by Drugs. Previously on May 18, 2016 under BPMC Order #16-170, the physician assistant was subject to a Non-Disciplinary Temporary Surrender of her New York State medical license.
  License Limitations or Conditions for DOH Webpage: During the period of probation the physician assistant may only practice medicine when her medical practice is being supervised by a licensed physician on site at all locations and she is being monitored by qualified health care professionals.
