 Action Description for DOH Webpage:This order is a modification of BPMC Orders #17-332 and #19-084 and does not constitute a new disciplinary action. As of the effective date of this modification order the physician shall be allowed to register his New York State medical license. Once registered and medical malpractice insurance is obtained the physician shall be allowed to practice medicine in New York State. The physician shall be limited to practice medicine solely within a group practice, which contains at least two other physicians in general surgery and/or colon and rectal surgery. The physician has completed his Clinical Competency Assessment and based upon all relevant facts the licensee’s order is modified. The physician’s New York State medical license limitation remains in effect. The physician must enroll in and successfully complete a program with the Committee for Physicians’ Health and also obtain a psychiatric consultation with a physician approved by the Director of the Office of Professional Medical Conduct. The physician must also enroll in and successfully complete two additional Continuing Medical Education programs. The physician is also placed on probation for thirty-six months with conditions and with the approval of the Director he must enroll and successfully complete additional medical credits per his modification order. The physician also remains subject to the terms and conditions of the previous board orders.
 Misconduct Description for DOH Webpage:This order is a modification of the terms previously imposed on December 5, 2017 under BPMC Order #17-332 and First Modification Order No. 19-084 which went into effect April 19, 2019, and does not constitute a new disciplinary action. The physician did not contest the charge of committing professional misconduct by failing to maintain accurate patient medical records.
  License Limitations or Conditions for DOH Webpage: During the period of probation, the physician may only practice medicine when his practice is being supervised by an on-site practice supervisor and when his practice is being monitored by a physician board certified in an appropriate specialty.
