 Action Description for DOH Webpage:Indefinite license suspension for no less than twelve months and the physician’s New York State medical license is permanently limited to preclude him from prescribing, administering, ordering or dispensing of controlled substances. During the period of suspension the physician is precluded from reliance on his New York State medical license to exempt him from the license, certification or other requirements set forth in statute or regulation for the practice of any other profession licensed, regulated or certified by the New York State Board of Regents, New York State Department of Education, New York State Department of Health or the New York State Department of State.The license suspension remains in effect until a Committee on Professional Medical Conduct finds that the physician is both fit and clinically competent to practice medicine. Upon such a finding by the Committee the physician will be placed on probation for five years with conditions and may be subject to additional terms and conditions deemed appropriate by the Director of the Office of Professional Medical Conduct.
 Misconduct Description for DOH Webpage:The physician did not contest the charge of having committed professional misconduct by having been disciplined by the New Jersey Board of Medical Examiners for having relapsed in his use of alcohol and for having been convicted in the Municipal Court of Monmouth Beach, State of New Jersey of Driving While Intoxicated.
  License Limitations or Conditions for DOH Webpage: The physician’s New York State medical license is permanently limited to preclude the prescribing, administering, ordering or dispensing of controlled substances. During the period of probation the physician may only practice medicine when his practice is being supervised by a licensed physician and when he is being monitored by qualified health care professionals.
